Yes, had to drill the top cover lid pieces. A template is furnished with the rack to do this and is fairly accurate. Take you time and get it just right, you won't be sorry. When the case is mounted, it's perfect to use as a passenger backrest. A backrest pad is available with the trunk but I didn't get it since I only ride solo.
The Twisted Throttle set-up is excellent. I put on a 56 liter case. Did have to remove the antenna with a case this large. Relocated it under the seat with a zip tie.
Yesterday I picked up a V47 Givi case, backrest and mounting kit at Twisted Throttle and after installing the back rack I really didn't like how it sloped upward, at the opposite of the bike's tail end that slopes down. With the case on it didn't look as bad but it still looked a bit odd to me. So I decided to remove the plate and use the shorter front bushing on the back of the mounting bracket. For the front I cut one of the long rear bushings in half and used them in the front. I put the plate back on and it slopes is in the same direction as the tail, the case sits closer to the bike and it looks way better.
Here's mine setup. This is a Honda trunk but made by Givi. I also had the backrest custom made for it.

Cool Hand Luke - just getting around to this project, what rack is under the Honda trunk and what size trunk would that be?
Trunk and Rack.... Attaches to the standard Honda rack for me.... GIVI 52 liter
that looks really good and keeps the weight down low. what was involved in getting that on?
